You are expected to attend classes for which you are registered. Each class instructor may determine the relationship of class attendance to the final grade for the course.
Faculty should tell you what relationship attendance has to your final grade for the course, but you must take the responsibility for your actions. Notification to teachers of emergency absences (death in the family, serious illness, etc.) is your responsibility.
BYU requires all continuing students who are enrolled at least three-quarter time (including Salt Lake Center hours) to have appropriate medical coverage for the duration of their enrollment at BYU. In other words, you must have coverage the entire time you are a continuing BYU student, including during any summers you take off or other short-term breaks from classes.

If you choose any medical plan other than the BYU Student Health Plan, you must provide verification of adequate coverage at the beginning of your first semester or term and annually at the beginning of fall semester. Carefully review any medical plan you are considering to make sure it will provide you with sufficient coverage for your current and future healthcare needs. Coverage must be effective by the first day of class.
All undergraduate and graduate students enrolled at least three-quarter time (9 credit hours per semester or 4.5 credit hours per term, including Salt Lake Center hours) who do not submit proof of other coverage through the Route Y Web site to the BYU Student Health Plan Office before the add/drop deadline each year will be enrolled automatically for individual coverage and assessed the appropriate premium (single or married student rate) for the BYU Student Health Plan.
Spouses and dependents will not be enrolled automatically the first semester or term that you are on the plan. If you want coverage for your spouse and dependents, you must enroll them in the plan. Once they are enrolled, your dependents will be enrolled automatically at the beginning of each subsequent academic year.
The BYU Health Plan is an annual plan but is billed and must be paid each semester/term. The health plan fee is due the first day of class each semester or term. Following fall semester, students who will not be attending BYU for one or more semesters or terms (except as described above) are eligible to remain on the BYU Health Plan but must continue to pay the health plan fee on a timely basis. Following fall semester, students who will no longer be attending BYU may also elect out of participation in the health plan. If you choose to not remain on the Student Health Plan, you must submit in writing such decision to the Student Health Plan Office by the due date of the plan fee.

Undergraduate students who desire to transfer from one major to another, or from one college to another, should go to the College Advisement Center of their new major, fill out a Change of Major Form, and have it signed. Graduate students who want to change departments or degree levels should check with the Office of Graduate Studies for information and necessary forms.
