Submitting Department Scholarships


To award department scholarships:

  1. Use the scholarship submission form to submit all scholarships—including CPO submissions.  Undergraduate and graduate scholarships must be submitted on separate forms.  Contact Connie Smith at 422-5205 for copies of the submission forms.
  1. Please completely fill out contact information.  This person will be contacted if there are any questions or problems with the submission.  For undergraduate awards, this is also the person to whom we will send a copy of the submission sheet once the awards are processed.
  1. All forms must be signed by the department chair or designated department representative.  This signed form must also accompany all CPOs.
  1. The college/department is responsible for notifying students of their awards in writing.  However, undergraduate students are NOT to be notified of their awards until after their awards are processed by the Financial Aid and Scholarship Office and the contact person listed on the form has received a copy of the original submission form. Any changes made will be indicated on the copy.  A university policy sheet or comparable department policy sheet should be included with each undergraduate award letter.
  1. When reviewing a student’s eligibility for scholarships, you may wish to consider the following:
  • Is the student on academic warning or probation?
  • Does the student have more than 160 credit hours?
  • Has the student already received more than 8 semesters of scholarship?

    Any of the items listed above will prevent a student from receiving a university scholarship, but departments are free to award these students if they wish.

  1. If you know that a student will be registered for fewer credits than the minimum requirement to maintain a scholarship (usually 14 hours / semester or 6 hours / term), please notify the Financial Aid and Scholarship Office by memo or email.
